Can't /OPER

OK, I've been trying for the past 10 hours or so to figure out why I am 
unable to /OPER up on my newly created Hybrid.

I've double and triple checked my settings and I don't understand why it 
won't accept the Password.  I've tried using plaintext & mkpasswd with 
no luck at all.  I've read as many googled cases as I can to try and 
determine what's wrong and nothing seems to be working.

Can someone please give me some clue as to what I can try next?

Any and all help here would be appreciated.

FreeBSD 4.10
Hybrid 7
